Sunshine Health Transition Support for Youth

In Florida, you can receive healthcare coverage through Sunshine Health’s Child Welfare Specialty Plan until the age of 21 if you are in extended foster care. This program is specifically designed to meet your needs. Sunshine Health works closely with you and your Community Based Care Lead Agency to facilitate your transition from adolescence to adulthood. Recognizing the obstacles you may face while preparing to transition out of foster care, Sunshine Health is committed to providing the necessary services and support.

Program Benefits

  • Over-the-Counter Medications/Supplies: Participants receive a $25 monthly allowance for personal hygiene products, sexual health items, first aid supplies, and more. A complete list of eligible items can be found on the Pharmacy section of the website.
  • Transportation: Young adults are provided with round-trip transportation three times a month for social activities and essential destinations, including work and post-secondary education.
  • Financial Support: The member rewards program, MyHealthPays®, assists with expenses such as cell phone bills and rent.
  • Transition Assistance: Eligible participants can receive up to $500 in services and items for necessities such as rental deposits, utility services, or household supplies through the one-time Transition Assistance expanded benefit.

Eligibility

To be eligible for this program, you must meet one of the following:

  • Must be between 18-21 years old
  • A resident of Florida
  • You are in Extended Foster Care

Staying connected with your local Community-Based Care (CBC) lead agency and knowing key contacts is essential. This knowledge empowers you to efficiently navigate your needs, especially when case managers or mentors transition out of their roles. CBCs can assist you with obtaining vital documents, such as your birth certificate and Social Security card, crucial for employment and education records.

Additionally, they maintain your foster care records, which you may need for various legal and personal reasons. CBCs also guide you through application processes for Medicaid, housing assistance, and other support services designed to ensure your well-being and success.
