YOLO | Sibling Visitation Program

The SIBLING VISITATION service is similar to one on one mentoring, as in, we (YOLO Mentoring) pick up the siblings that are currently not placed together and have them spend time together in the community for 2 hours. We can service up to 4 kids in our car at a single time based on the number of seat belts in a standard car. We do not transport any child that needs a car seat.

Who Can Apply

  • A Case Manager must complete the referral form.

Deadline

  • No deadline. Referrals are accepted on a rolling basis.

Complete the Referral Form

Fill out all required fields on the referral form accurately. This information is essential for processing and evaluating the referral.

Scan the Completed Form

After filling out the form, scan it to create a digital copy. Ensure the scan is clear and all information is legible.

Email the Form

Send the scanned form to JAdorno@yolomentoringllc.org. Make sure to include any additional documentation or information requested on the form.

Review and Processing

Once received, the form will be reviewed and processed by the team. They will assess the information and determine the next steps based on the referral details.
