After Care Service

The After Care Service program is dedicated to offering you ongoing support as you transition out of foster care, ensuring you continue to receive the stability and guidance needed to thrive in adulthood. This program is designed to bridge the gap between foster care and independent living by providing crucial resources such as mentoring, counseling, and life skills training. By maintaining a connection with you as you age out of the system, After Care Service helps you navigate the challenges of adulthood with confidence and security. It serves as a vital safety net, offering personalized assistance that empowers you to build a stable and successful future. Through this program, we are committed to ensuring that you do not have to face the journey to independence alone.

Eligibility

To be eligible for this program, you must meet one of the following:

  • You turned 18 while in the Department’s custody or spent at least 6 months in foster care after turning 14 without being reunified with a parent or guardian.
  • You are a Florida resident.

Helpful Tips

  • Create a list of services you may need and contact aftercare programs to see what they offer.
  • Maintain regular communication with your after case manager or IL. Specialist to stay informed about available resources.
  • Take advantage of counseling and mentorship programs to help guide your decisions.
  • Make sure to provide any updates on changes in your situation and complete necessary renewals.
  • These services are available until your 23rd birthday.

This knowledge empowers you to efficiently navigate your needs, especially when case managers or mentors transition out of their roles. CBCs can assist you with obtaining vital documents, such as your birth certificate and Social Security card, crucial for employment and education records.

Additionally, they maintain your foster care records, which you may need for various legal and personal reasons. CBCs also guide you through application processes for Medicaid, housing assistance, and other support services designed to ensure your well-being and success.
